Of course, we will take the time during your visit to discuss these in further detail with you. My child did not pass the vision test at school/pediatrician's office. What should my next step be? A comprehensive eye exam is recommended to rule out the need for glasses and any ocular abnormalities. Any vision or ocular concerns should be treated early to ensure proper visual development. Please schedule an appointment with an optometrist or ophthalmologist focusing on children's vision. What is the difference between an ophthalmologist, optometrist, and optician? Ophthalmologist are physicians (M.D.) specializing in the eyes. After earning a Bachelor's Degree, they attend four years of medical school then complete an additional four year residency focusing on the eyes. Ophthalmologists are trained to perform primary care eye exams, treat all eye diseases and perform eye surgeries. Ophthalmologists work with optometrists to ensure the best glasses and contact lens prescription for the patient. Optometrists (O.D.) are primary care eye doctors. After earning a Bachelor's Degree, they attend four years of optometry school to learn about the eyes. They are trained to perform primary care eye exams, treat certain eye diseases, and have extensive training in glasses and contact lenses. Optometrists work with ophthalmologists to care for patients who require eye surgery or have eye diseases associated with a medical condition. Opticians are trained to fit patients with the best glasses possible from the prescription provided by an optometrist or ophthalmologist. They are knowledgeable in the different lens and frame designs available. Some opticians are also prepared to teach patients how to utilize contact lenses. What should I expect at my child's first eye exam? Foremost, a medical history form will be completed for the examining doctor to review. Your child will first see the medical assistant (MA) who will help start the exam. The MA will take an initial measurement of the eye behind a machine called an autorefractor – this gives an estimate of your child's focusing error. Then, vision at distance and near, depth perception, and color vision will be measured. When the child sees the examining doctor, eye alignment, prescription measurement, and eye health evaluation will be completed. The doctor may use a phoroptor (big mask with lenses inside) or a retinoscope (special flashlight) to help measure whether glasses are needed. A microscope and special magnifying lenses help to evaluate the health of the eyes. Eye drops may also be indicated to complete the exam. What is a cycloplegic exam? Why should my child be dilated? Eye drops are used to help us to further evaluate vision and eye health. Firstly, the eye drops will cycloplege (fully relax) the focusing muscles of the eyes, allowing for a more accurate glasses measurement. This is especially important for children in the treatment of amblyopia (lazy eye) and strabismus (eye turn). Secondly, the eye drops will also dilate (enlarge) the pupils. This will allow us to evaluate the health of the eyes. This is important to rule out any eye diseases or abnormalities that may be affecting your child's vision. What is amblyopia (lazy eye)? Amblyopia is reduced vision in one or both eyes due to uncorrected vision conditions early in life. If the brain does not receive maximal visual input during early childhood, vision does not fully develop. Amblyopia is best treated early while the brain is still developing vision. What is strabismus (eye turn)? Strabismus is when the eyes are misaligned. Eyes can turn in, out, up, or down. One or both eyes can be affected. Eye turns can be present constantly or intermittently. Strabismus can lead to amblyopia.. What is patching? Why does my child need to be patched? Patching is used to help treat amblyopia. In many cases, glasses are prescribed and this alone resolves the amblyopia. However, in cases where vision does not improve with glasses alone, or the vision improves asymmetrically, patching is then recommended. Patching completely occludes the stronger eye in order to force the weaker eye to be visually stimulated. What is myopia (nearsightedness)? Myopia is a focusing error where objects far away are blurry, but objects up close are clear. This is not a disease and is correctable with glasses / contact lenses.

What are Cataracts? A cataract is a progressive clouding of the natural lens of the eye. This clouding interferes with light as it passes to your retina. This leads to deterioration in the quality of your vision. Symptoms include the sensation of looking through a piece of wax paper, distortion or dimming of vision and color. This can interfere with your reading and driving. Some forms of cataract, including cortical cataracts, do not cause much effect on reading vision in normal lighting conditions, but with headlights with night driving and trying to read on a bright day these cataracts can lead to significant glare and haloes. Currently there is no medical treatment to reverse or prevent the development of cataracts. Once they form, the only way to achieve clear vision again, is to physically remove the cataract from your eye. Advances in Cataract Surgery 1. No injection surgery In traditional cataract surgery, a long needle is inserted behind the eye to numb it for surgery. This "retrobulbar" or "peribulbar" injection has a lot of risks, including injury to the eye or the eye muscles and nerves from the needle, bleeding or infection. Dr. Kaur uses a newer and safer technique with advanced medical drops. In this way, the risks of injecting medication behind the eye with a needle are avoided. In addition, the recovery from surgery can be greatly improved. 2. No suture surgery In traditional cataract surgery, a large incision, the size of the whole lens is made in the sclera (white part of the eye). The lens is then removed in one whole piece. Following this, the incision must be closed with multiple stitches. This very large wound may increase the risk of complications, including bleeding and infection. In addition, the sutures can lead to a permanent distortion of vision from irregular astigmatism. After surgery, a patient's activities may also need to be severely restricted to prevent increased pressure in the eye in the setting of a very large and potentially unstable wound. Dr. Kaur uses the latest surgical techniques and instruments to make microincisions in the cornea (clear part of the eye). These incisions are self-sealing and so no permanent sutures are needed. Some patients may need one temporary suture. This is removed in the office 1 week after surgery and does not result in any astigmatism. 3. Phacoemulsification Dr. Kaur does all of her cataract surgeries in only accredited surgery centers or hospitals with the latest cataract surgery equipment. She is trained in the most advanced techniques. These include phacoemulsification, where Dr. Kaur uses ultrasound energy to liquefy the cataract and then remove it through microincisions in the cornea. Advances in Lenses After the cataract is removed from your eye, it is replaced by an intraocular lens which helps you see clearly after surgery. Older lenses are made of hard plastic and must be inserted through a larger wound, which needs to be closed with sutures. Dr. Kaur uses foldable lenses made of the newest lens material of acrylic. These lenses are folded with a special instrument which can be inserted through microcincisions in your cornea. This leads to less distortion of the cornea from surgery. Aspheric premium lenses Dr. Kaur uses aspheric lenses. These improve contrast sensitivity and have less visual aberrations (distortions in vision), then regular spherical lenses. Multifocal Premium lenses You now have the potential to see clearly at both near and far without glasses after cataract surgery. • Lipiflow for Dry Eyes

  Dry eyes is a condition where the natural tear of the eyes are unable to sufficiently lubricate and moisten the surface of the eyes. Dry eyes can affect both vision and comfort; thus, compromising quality of life. Dry Eyes syndrome affects more than 100 million people throughout the world. Now, a revolutionary Dry Eyes therapy, LipiFlow, is available at California Eye Surgeons to treat one of the most common underlying causes of Dry Eyes! Dry Eyes The tear film which coats the front surface of the eye is made up of three layers. The mucin layer adheres tears to the eye's surface, the aqueous (water) layer nourishes and protects the eye, and the lipid (oil) layer lubricates and prevents the tears from evaporating. Each of these layers plays an essential role in maintaining tear film integrity. If any of these layers are affected, the sensation of dryness can result. Tear Layers Symptoms of Dry Eyes may vary by Individual. Patients may notice one or all of the following: redness, irritation, stinging/burning sensation, sandy/gritty feeling, blurred vision and watery eyes. Dry Eyes can impact daily events from reading the working on the computer to being able to wear contact lenses comfortably and enjoying outdoor activities, especially on a windy day. LipiFlow The two primary types of Dry Eyes are Evaporative Dry Eye and Aqueous Dry Eye. Aqueous Dry eye is associated with insufficient tear production or poor tear quality. Evaporative Dry Eye is associated wtih degeneration or deficiencies of the meibomian glands, which are responsible for producing the oil layer of the tear film. Evaporative Dry Eye affects approximately 85% of patients with Dry Eyes. LipiFlow is an innovative treatment that addresses the underlying cause of Evaporative Dry Eye. This thermal pulsation therapy applies controlled warmth and pressure directly to the inner eyelid and meibomian glands. This process helps to release oils that are clogged in the meibomian glands allowing for a more normal flow of oils onto the tear film. Additionally, it helps to increase the action of defected glands to prevent the complete loss of gland function. • Laser Surgery for Glaucoma

  Dr. Kaur uses the most advanced Lasers to treat the underlying conditions that contribute to elevation of pressure within the eye. Open-angle glaucoma is treated with Argon Laser Trabeculoplasty and Selective Laser Trabeculoplasty. The YAG Laser peripheral iridotomy procedure can be performed to treat closed angle glaucoma.

  Upper and lower eyelid surgery can improve vision from sagging eyelids and improve the overall appearance of eyes and face. For upper eyelid surgery excess skin and fat can be removed through a small incision hidden in the natural eyelid folds. For the lower eyelid surgery Dr. Kaur makes a small incision on the inside of the eyelid. Most patients can return to normal activities within a week.
